Checkout questions on phone order?

Is it possible to have the checkout questions we added on the phone orders? I couldn't find anyway to have them added.

Not at this time, however, its in the "todo" list. We are also going to bring the shipping insurance option to phone orders which is currently not available.

Also in the phone order if the customer has shipping addresses in their aaddress book you can't pick one of the other addresses.

And is there a way they can save a shipping address to their account when they place an order?
