Special Order Products

We are getting ready to go live with our site on 1-1-08 and have one, final (minor) piece to migrate. We could never figure out how to do this on Monster, so it is still with our old-old cart, Mals-E Commerce.

We do a lot of custom work and special order products that we don't have on our website. A lot of these orders are taken via e-mail, so I designed the above page to let the customer pay securely using our regular card processing.

I set up a quote in Stone Edge and e-mail them the total, the quote number/description and they use the page above to pay for their order.

So, my question: Any idea how to do a product/page like this on 3dcart?

I mostly need the freedom to allow the customer to enter any price. They could put the other info in the order notes at checkout.
