Help! Free shipping config!

How do you config the cart to only offer free shipping on orders to the US?

I created a custom ship option, added 0% markup, excluded int'l, but it will not show up in a shipping option. I swear it was working 3 days ago!

Free Shipping would be a "promotion" set up in the promotions menu.

So you would set up a regular shipping method though the shipping menu. Then create a free promotion for people living in the US.
