Just wanted to give a big thumbs up to JotForm.com for those who are code challenged, as I am, and need an easy solution for creating a form. I created one in about 5 minutes and it fits beautifully within my site. I'm not associated with them in any way, just a satisfied customer. I'm only using their free set up for now, will wait to upgrade based on performance.
